TMC Car Seat Loaner Rental Agreement

Description

TMC's Car Seat Loaner Program is open to anyone in the community who needs to borrow an age and weight appropriate car seat for up to 1 month. A standard rear facing only car seat rental is $50 and a SPICA car seat rental is $100. You will be asked to select one during check out.

**Note** Please do not complete this unless told to do so by a staff member at TMC. If you are outside of TMC and need to borrow a car seat, please email me at jessica.mitchell@tmcaz.com.
